What To Bring To Your Appointment
Welcome to Mountain Home Podiatry. We want to take good care of our patients, and help answer some of your questions in advance. 
  • ID card
  • Insurance card
  • Medical notes related to your condition (Charts / X-rays / MRI) if you have them.
  • Completed Patient Intake Form. For your convenience, print this form from the website and complete at home, or complete at the office.
  • Payment (We accept checks, cash, credit card, Health Savings Account card)

We accept most insurance. We also accept HSA cards, plus we offer cash prices for patients without insurance. If you do not have insurance and plan to pay cash for our services, here is a list of common services we offer. Call the office for an estimate on pricing - 870.580.5589. Payment is due in full at the time of service.
